Corresponds to FHIR CareTeam participants

Being a participant in a care team grants access to the subject (a member). This is relevant in the case where a practitioner should not have full access to all medical data.

Each participant can have roles in the care team. It's also possible to have no roles. These roles bring extra permissions that can be configured at a project level. For example: a pharmacist may have access to prescriptions, but not the lab results, while a doctor may have access to everything.
