Care Plan

A care plan describes the intention of how one or more practitioners intend to deliver care for a particular patient for a period of time, possibly limited to care for a specific condition or set of conditions.

A care plan typically refers to a care plan template, but it can also be custom-made for one specific patient. A patient may have several active care plans. Once a plan is actually carried out, it typically results in other resources like Appointment, QuestionnaireResponse, Observation, ...
